一、依據「特殊教育法第 36 條」、「特殊教育法施行細則第 10 條」及「各教育階段身心障礙學生與幼兒轉銜輔導及服務辦法」辦理。
二、請各校(園)於每學年度第二學期末完成「臺南市高級中等以下學校身心障礙學生與幼兒轉銜輔導及服務檢核表」留校(園)備查，本局或特殊教育資源中心等中心得以電話、書面或實地訪查等方式進行督導。
三、請務必進行鑑定安置程序，再依流程完成轉銜相關作業，並確認至教育部特殊教育通報網填寫轉銜服務資料之正確性及完整性。
